This page contains all of the forms that you need for club enrollment. These are NEW! The member Registration form has taken the place of the "Club Enrollment form".
One thing to note is that there is a PDF version and a Word version. Leaders/Parents, you can fill them out and save them on your computers so you don’t have to fill them out each time or for each club they are in, just change the club information and sign the new copies. Club leaders need to keep a copy for their records and submit the another copy to the office. The leader will only need to submit their enrollment form (it lets you know who is in the clubs) and the member enrollment forms.
